Update your Contact Information

To update your contact information, enter your Claim ID (from original email/postcard notice) or phone number that was used to file your claim.

Your Claim ID is located in the email or postcard notice that you received notifying you of this settlement. It is also located on your check stub, upon receipt of your settlement award. This is different from your Claim Submission ID that you received upon submitting a claim.

The purpose of this form is to update your contact information if you've already submitted a claim. For security purposes and to protect claimant information, this form will not pre-populate contact information or confirm receipt of a claim.
